This Black Paper, developed by Measure in partnership with the Episcopal Health Foundation, centers the voices of rural Texans navigating pre-diabetes and type 2 diabetes. Through a series of co-designed focus groups, community partners shared their lived experiences with healthcare access, cultural barriers, and the daily realities of managing chronic illness. The report offers quantitative findings and community-led recommendations that call for bilingual education, culturally grounded care, and stronger peer support networks. This is lived experience data—collected, honored, and used to drive systems change in health equity.
